Tooth Extraction for Pain Management

Tooth extraction is used when a tooth cannot be predictably preserved or when removal is necessary for another treatment plan. Our preference is to keep a natural tooth when it has a reasonable long-term prognosis, so extraction should follow a diagnosis and a discussion of realistic alternatives.

Why might a tooth need to be removed?

Common reasons include a nonrestorable fracture, extensive decay, severe periodontal bone loss, certain infections, failed prior treatment or a tooth that interferes with an orthodontic or surgical plan.

Can the tooth be saved?

Sometimes. Depending on the problem, alternatives may include a filling, crown, root canal treatment or periodontal therapy. A tooth with a poor structural or periodontal prognosis may not benefit from extensive treatment simply to delay extraction.

What happens during an extraction?

The area is thoroughly numbed. Some teeth can be removed with a straightforward extraction while others require a surgical approach. Complexity depends on root shape, fracture pattern, bone anatomy and access. Referral to an oral surgeon may be recommended for selected cases.

What should you expect afterward?

A blood clot forms in the socket and is important for healing. Postoperative instructions generally focus on controlling bleeding, protecting the clot, keeping the area clean and gradually returning to a normal diet. Increasing swelling, fever, uncontrolled bleeding or severe worsening pain should be evaluated.

Does every extracted tooth need to be replaced?

No. Wisdom teeth, for example, are not replaced. For other teeth, replacement may help preserve chewing function or prevent unwanted tooth movement. Options include a dental implant, bridge or removable prosthesis depending on the location and overall treatment plan.
